Medical (MED)

24/7 coverage nationwide from Etiqa’s medical network of doctors, hospitals, and clinics for no-cash-out access — extending to all present and future principals and dependents upon enrollment.

Benefits include inpatient, outpatient, and emergency treatment up to your Room & Board / Maximum Benefit Limits per disability per year.

  • No inner limits — includes medicines (not vaccines) administered during outpatient visits, and emergency ambulance service to and from anywhere, anytime
  • Access to non-accredited facilities worldwide for reimbursement as-charged, up to MBL

JuanHealthcare Corporate medical coverage is all-risk: whatever is not excluded is included. MED is charged to premium based on members’ R&B/MBL and the client’s current pool number. LOG is required for no-cash-out; no LOG is required for reimbursement.

A. Dental (DEN)

Nationwide dental benefits from Health Partners Dental, including:

  • • Simple adjustment of dentures

No LOG required. DEN is charged to premium at ₱300/head; dental claims are not added to your MED utilization. (No reimbursement allowed.)

B. Annual Physical Examination (APE)

APE is charged to premium at ₱1,500/head.

C. Life & Accident (L&A)

Worldwide life & accident from Etiqa equal to 1–5× MBL (₱1M max / NEL). Group Yearly Renewable Term (GYRT) includes:

  • • Death by any cause (except suicide in the 1st year)
  • • Total & Permanent Disability (TPD) pays in lieu of Life, after 6 months of TPD from sickness or accident
  • • Personal Accident (PA) includes Accidental Death & Dismemberment (AD&D), with indemnities for permanent disablement
  • • Murder & Assault (M&A) pays in lieu of accidental death & disablement — provoked or unprovoked

Separate GYRT/TPD/PA policies, riders, and exclusions apply. No over-age is allowed for L&A, even if allowed for MED.

The following are not covered under standard medical benefits unless otherwise stated or covered by optional riders.

  • Expenses that should be taken care of by government programs such as PhilHealth
  • Services rendered, or supplies provided, free of charge
  • Incremental costs from taking a higher room accommodation than the member’s R&B limit
  • Non-recommended confinement; convalescent/domiciliary/custodial care; rest cures
  • Medico-legal fees, including the cost of medical certificates
  • Procedures that are not medically necessary, or that are used for screening purposes only
  • Purchase of artificial aids, medical equipment & supplies, including nebulizer/suction/CPAP machines, thermo-scan/pulse oxy/BP devices, and all test kit supplies
  • Dental-related cases, except for optional dental benefits and cases of accidental injury; vision-related cases (errors of refraction), except for Lasik to treat legal blindness
  • Annual physical examinations, except for optional APE benefits; executive check-ups
  • Take-home or prescribed medicines; vaccines, except for anti-venom/rabies/tetanus
  • Maternity/maternity-related conditions/complications; fertilization/sterilization/sex change
  • Cosmetic/aesthetic procedures, except for reconstructive surgery for functional defects
  • Complementary & alternative medicine (CAM) — medical systems, practices, and products that are not mainstream but associated with good health
